The film opens attempting to build character of two young couples. The next thing you know they are trapped inside a house with other people. Their brains are connected to a collection system which measures and deposits chemicals produced when the body is in pain. The captives have 22 hours (86 minutes movie time) to produce the desired amount of this chemical or else! They can feel free to use any of the torture devices provided.
There are two things you can do, should you want to play the game.
A) Figure out which ways effectively get the desired chemical by causing pain, but minimizing the damage to the body, such as pulling a tooth and irritating the nerve ending underneath or... B) Draw lots and randomly torture each other in different ways.
Plan B makes for a better film. The film was mindless and senseless, not like the original "Saw." Unless you just want to watch people hitting and cutting each, you might want to take a pass. Oh yes, expected twist blah blah blah. The opening soundtrack/scene was the best aspect of the film.

Plot summary
A group of friends stop to pick up a hitchhiking woman only to end up getting drugged by her with a gas. They awaken to find that vials have been implanted in the base of their skulls - which are of course instantly fatal if they are removed, a grinning professional looking woman informs them on TV screens that they have 22hrs to fill these vials with a specific amount of brain fluid, a fluid that is produced during times of extreme pain. Along with another group of unlucky test subjects and with time ticking away they decide to work together and share the burden of reaching their painful target.

A film where people are forced to torture each other? With the dubious reason that brain chemicals are being collected? While there is some cleverness to this latter part, the former boils down to yet another title in the never-ending "torture porn" subgenre of horror. And no one wants that. Of course, viewers should not be surprised when the cover is a pliers holding a tooth.
If there is one redeeming thing about this film, it is the inclusion of Maynard James Keenan as Special Agent Ford. But, to be fair, the torture aspects are stronger in the first half and there is some attempt to bring out the humanity in these folks. Does that make this is a good movie? No. But it could be a lot worse.
Horror falling flat
With a threadbare and straightforward plot, if done right it could have worked. The premise of the kidnapping gives could have given a nice place to begin, but the writing took the plot nowhere. Just the simple "torture each other and you'll get out" for the rest of the plot doesn't give anything to work with.
The characters are not developed other than who are intended to be the loudest and bossiest and are all poorly acted. There was far too much time spent on the most annoying character just bitching, screaming and making excuses. About the only thrill I got from the movie was when she was knocked out by a punch and getting her just rewards.
